All summer camp participants must complete the required forms two weeks before camp.

All participants must complete the Camp Participant Required Information Form.

If your child will need to receive medication during camp hours please also print, fill out and return the Authorization for Administration of Medication Form.

If you are interested in purchasing box lunches prepared by Ocean Blue Catering for while your child is at summer camp please print, fill out and return a Box Lunch Order Form.
